Application essentials
- Appointments must be booked through Prenot@Mi (prenotami.esteri.it). Slots are released unpredictably and fill within seconds.
- All non-Italian vital records require an apostille from the issuing US state, plus a certified Italian translation.
- The appointment fee is $703.10 USD per adult applicant (locked rate through June 30, 2026), paid by cashier's check or money order on the day of the appointment.
- Consulates do not provide genealogical research or document translation services — you must source these yourself.
Office-specific notes
- Forms 1 and 2 must be signed in person before the Consular Officer on the day of your appointment — do not sign them in advance.
- Form 3 requires a living ancestor's signature. If the ancestor cannot appear in person, their signature must be notarized (or, if they live outside the US, legalized by the competent Italian consulate).
- Form 4 is only required when a direct-line ancestor is deceased and was born outside Italy.
